It doesn't hurt so bad. A little pinch, the jewelry goes in and it's over. Remember to breathe! Take a deep breathe in through your nose and exhale through your mouth slowly while they pierce you, it really does help.
I have only had one piercing done with a clamp and that was a helix. The clamp definitely hurt more than the needle because it was just uncomfortable. Who I go to now does it all freehand, so I wouldn't know what it would have felt like on my stomach.
The anaesthetic you would have to ask whoever is piercing you. It really does not hurt that bad.
